  • The name and address of your employer(s) from this year and last
  • A copy of your Social Security Statement
  • The beginning and ending dates of any active U.S. military service
  • A list of jobs and dates you worked over the last 15 years before you became disabled
  • Information about workers’ compensation or similar benefits you filed or intended to file for
